Sending your insurance card and photo ID allows us to verify coverage, streamline scheduling, and reduce delays before your first session. 

Release of Information (Optional)

Completing a Release of Information allows coordinated care and communication with other providers, ensuring support is aligned, informed, and centered on your wellbeing.

Secure Credit Card Authorization Form

Step 3: Complete authorization to put a credit card on file.

 

This form allows you to safely authorize charges for agreed-upon services or purchases. All information is handled with care and stored securely for future transactions on your account. You may cancel this authorization at any time by contacting us directly.
